In addition, UW Medicine shares in the ownership and governance of Childrens University Medical Group and Seattle Cancer Care Alliance.

The Seattle Cancer Care Alliance (SCCA) Oncology Pharmacist team provides extensive pharmaceutical care to inpatient and ambulatory patients. In addition to direct care, pharmacists participate in drug therapy consults, conduct clinical research protocols, provide patient/family/staff education, and precept pharmacy students and residents. SCCA combines the adult and pediatric oncology patient care services of three world-renowned cancer care institutions Fred Hutchinson Cancer Research Center, Seattle Children's, and UW Medicine.

SCCA has an outstanding opportunity for a TEMPORARY/HOURLY PHARMACIST 2 - IV ROOM (Rotating/Variable Shifts) to join the Oncology Pharmacist team. The Pharmacist 2 will perform professional pharmaceutical dispensing and clinical functions in the Ambulatory Pharmacies. Additional duties include: providing drug information and education to patients and family members, providers, nurses and other health-care professionals; and providing pharmaceutical care to patients.

Primary responsibilities:
Conduct a thorough patient profile review to assess the appropriateness of the prescribed medication, including monitoring for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received; document relevant information in medical record as required.
Promote rational, cost-effective drug therapy by promoting the use of formulary items and optimizing patient care through appropriate selection of treatment options.
Evaluate all provider orders and verify correctness of entry in the pharmacy computer system and computerized patient profiles.
Direct pharmacy technicians in all areas of technical responsibilities.
Ensure proper preparation and labeling of all medications.
Ensure that controlled substances are properly stored, dispensed, recorded, and inventoried.
Interview patients and provide education regarding their medications.
Ensure that all bulk packaged items are properly prepared, labeled and documented.    
Ensure correct aseptic preparation, labeling and delivery of compounded sterile products.  Perform routine visual observation and oversight to ensure proper garbing, hand washing, cleaning and aseptic technique processes is followed during sterile compounding.
Provide drug information to patients, physicians, nurses, and other members of the healthcare team. Provide in-services to staff as requested.
Participate in protocol development, exploratory projects and committees which contribute to improved patient care.
Document clinical interventions as appropriate.
Detect, monitor, document and report any adverse drug reactions and medication errors.  Assist in developing safe medication practices to prevent errors.
Participate in teaching pharmacy students, interns and technicians and guiding experiential training.
One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.

LEGAL REQUIREMENT: State of Washington Pharmacist license.
